Print and Complete this form and bring with you to your appointment along with a photo ID and your insurance cards.

It is now possible to send confidential messages to our office in regards to clinical questions, appointments, billing, medications, test results, referrals, and other issues. Log onto your portal account and click "create message" in the upper left corner. Choose the person to whom you want to send your message, enter the reason for your message (in the “Re:” field) and the subject of your message, then type your message into the “Message” field. You will get a response within three business days.
